I think this is a pretty good idea. However I'm sure that it won't satfisfy every requirement. I did something similar to this (although not quite as advanced) back in the late 90's and it worked well for that particular company.
I like the idea of having one form for each type or category of information. I can see that this would be very useful in many apps.
