Error: 'Validator.form' message

I found this code and think is wonderful, just what I needed. I followed the
instructions and copied the Save button, but get Error: 'Validator.form' is
null or not an object. I'm not sure what I am missing. Any help is
appreciated. (Sorry, I posted to the wrong place the first time).
