Synopsis
Tried to reinstall Notes 6.5.4 twice after the error occured. Still happening as I launch Notes.

This error is because of the design template failure in lotus notes.
Just find out your server name and then click on file > database > replace design.
Type in your server name and select your template name (lotus notes R6).
Then click on 'Replace' wait for 10 min, close lotus notes, delete cache.ndk, cache.ntf files and relaunch your lotus notes and that will the issue.
